MEDICAL SOCIAL-WORK
  • Assess and evaluate psycho-social needs

  • Evaluate environment for safety concerns

  • Provide additional community resources

  • Facilitate state Medicaid/waiver applications

  • Assess for long-term care (LTC) needs and coordinate LTC planning

RESPIRATORY THERAPY
  • Assess and evaluate respiratory functions and limitations

  • Perform diagnostic testing to determine respiratory status

  • Work with physician care team to develop treatment plans

  • Set-up and maintain Bi-PAP and C-PAP 

  • Use specialized treatments such as chest physiotherapy and aerosol medicals to target respiratory conditions

  • Monitor and record progress, making informed changes to reach targeted goals
